I've been messing around with this article on notes.net :

trying to get this Feedback form to work. I can't seem to get it all right and am finding the instructions inadequate. I'm not experienced at programming, but nonetheless i'm trying to jump in here, learn and fight through it.
I have my form set up correctly but I cant see where I'm supposed to instruct the Form to mail in to the suggested database.
They also suggested that after the user pushes submit, it should thank them for their submission. I also can't get the form to closeout correctly.
I'm just frustrated, but I think it would be quick and easy for someone quicker than myself.
If I am not being too forward, would someone be willing to help me through the creation of the feedback form?
